What this document covers:
Safety & Risk: Volleyball involves physical risk. By registering, you acknowledge that and agree RTB Volleyball is not liable for injuries that occur during normal participation.
Emergency Medical: If we can't reach you in an emergency, you're authorizing us to call 911 and consent to emergency treatment on your child's behalf.
Photos & Video: We photograph and film at our events for use on Instagram, our website, and other RTB marketing. This is a condition of participation.
Conduct: Participants are expected to be respectful and coachable. We reserve the right to remove anyone from a session whose behavior puts others at risk.

RTB VOLLEYBALL PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND CONSENT FORM RTB Sports LLC

PLEASE READ THIS ENTIRE DOCUMENT CAREFULLY BEFORE SIGNING.
This Waiver, Release of Liability, and Consent Form ("Agreement") is entered into by the undersigned parent or legal guardian ("Parent/Guardian") on behalf of the minor participant identified below ("Participant") in connection with participation in any RTB Volleyball program operated by RTB Sports LLC ("RTB Volleyball"), including but not limited to open gyms, skill clinics, and tryout sessions. By completing registration and checking the box below, Parent/Guardian acknowledges that they have read, understood, and voluntarily agreed to all terms set forth herein.
1. ACKNOWLEDGMENT OF INHERENT RISK
Parent/Guardian acknowledges and agrees that participation in volleyball and related athletic activities involves inherent risks of injury, including but not limited to: - Sprains, strains, fractures, and other musculoskeletal injuries - Cuts, bruises, and abrasions from contact with the court, ball, or other participants - Overexertion, heat-related illness, or physical exhaustion - Collision with other participants, equipment, or facility structures - Concussion or other head injuries - Aggravation of pre-existing conditions Parent/Guardian acknowledges that these risks exist even when RTB Volleyball exercises reasonable care, and that RTB Volleyball cannot guarantee the safety of all participants at all times.
2. RELEASE OF LIABILITY AND WAIVER OF CLAIMS
In consideration of Participant's opportunity to participate in RTB Volleyball programs, and to the fullest extent permitted by the laws of the Commonwealth of Virginia, Parent/Guardian, on behalf of themselves, Participant, and their respective heirs, assigns, and legal representatives, hereby releases, waives, and discharges RTB Sports LLC, its owners, directors, officers, employees, coaches, volunteers, agents, and representatives (collectively, "Released Parties") from any and all claims, demands, causes of action, damages, losses, costs, and expenses of any nature arising out of or relating to Participant's participation in any RTB Volleyball program, whether caused by the negligence of the Released Parties or otherwise, except in cases of gross negligence or willful misconduct. Parent/Guardian further agrees to indemnify and hold harmless the Released Parties from any claims, including reasonable attorneys' fees, arising from Participant's participation in RTB Volleyball programs.
3. EMERGENCY MEDICAL AUTHORIZATION
In the event of an emergency in which Parent/Guardian cannot be reached in a timely manner, Parent/Guardian hereby authorizes RTB Volleyball staff to: - Call emergency medical services (911) on behalf of Participant - Consent to emergency medical treatment, examination, or transport deemed necessary by medical professionals - Provide emergency personnel with the medical information disclosed at registration Parent/Guardian acknowledges that RTB Volleyball is not responsible for the cost of any emergency medical treatment and that this authorization does not obligate RTB Volleyball to provide or arrange medical care beyond contacting emergency services.
4. PHOTOGRAPHY, VIDEO, AND MEDIA RELEASE
s a condition of participation, Parent/Guardian grants RTB Sports LLC a perpetual, royalty-free, worldwide license to photograph, record, and use the likeness, image, name, and voice of Participant in connection with RTB Volleyball's programs, marketing, and communications, including but not limited to: - Social media platforms including Instagram, Facebook, TikTok, and YouTube - The RTB Volleyball website (rtbvolleyball.com) - Promotional materials, flyers, and advertising - Program documentation and highlight content RTB Volleyball will not sell Participant's likeness to third parties. This release is a condition of participation and there is no opt-out. If Parent/Guardian does not consent to this release, Participant may not participate in RTB Volleyball programs.
5. CODE OF CONDUCT
Parent/Guardian acknowledges that Participant is expected to conduct themselves in accordance with RTB Volleyball's standards at all times, including: - Treating coaches, staff, and fellow participants with respect - Following all instructions from RTB Volleyball staff - Refraining from conduct that is dangerous, disruptive, or disrespectful RTB Volleyball reserves the right to remove any participant from a session or program without refund for conduct that violates these standards or poses a risk to others.
6. GOVERNING LAW AND SEVERABILITY
This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia, without regard to its conflict of law principles. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
7. ENTIRE AGREEMENT AND VOLUNTARY EXECUTION
Parent/Guardian acknowledges that they have had the opportunity to read this Agreement in full before signing, have had the opportunity to ask questions, are signing voluntarily and without duress, have authority to sign on behalf of the minor Participant named above, and are the Participant's parent or legal guardian.
